Alternative testing arrangements for West Campus

Accommodation Request form forwarded to instructor
An Accommodation Request form will be forwarded to the instructor. This memo verifies that disabilities services has documentation of the disability. The form will specify the accommodations requested, including the alternative testing.
Discuss testing arrangements with instructor
Please take time to discuss the special testing arrangements you will need with your instructor.>Determine whether you will be allowed extended time in the classroom or whether you will be taking the test in disabilities services.
Schedule an appointment to test in disabilities services
Make an appointment to test in disabilities services with Ron Thiessen at 727-816-3236. Schedule the appointment at least five days in advance and indicate what type of accommodations you will need (extended time, reader, etc.).
Deliver test envelope to Instructor
You will then be given an empty, specially marked, test delivery envelope to deliver to your instructor. The instructor will complete the form on the front of the envelope, specifying proctoring conditions, and will deliver the test to disabilities services. Please see that your instructor receives the test envelope promptly, so that she or he will have time to deliver the test to the office.
Know and follow testing procedures
Your test appointment should correspond to the day and time that your class is taking the test.For example, if your class begins at 9 a.m., your appointment time must also be 9 a.m., unless otherwise specified by your instructor.

You must complete the entire test on the day of your test appointment. Once the test is turned in, it will not be returned to you. If you suspect that you will not be able to complete your test in one sitting (because of a following class or the length of the test), you must let the office staff know before we hand you the test so we can divide the test. We will divide the test only with the approval of the instructor.

If you do not make an appointment to take your test, or if you miss your test appointment, it is your responsibility to contact your instructor.
